- STUDENT IMMUNIZATION
- The immunization schedule listed below is required by the State of Texas for all students enrolling in any Texas public or private school. Please note that by certain grade levels, students must have measles or mumps vaccines or a written verification that the student has had those illnesses.
- Parents should submit complete documentation of immunizations showing the day, month, and year that each immunization was received. Those documents must be validated by or for a physician or public health professional with a signature or stamp.
- Please check the records you submit to make sure your student’s documented immunizations comply with the schedule below. Remember, a complete immunization record must be submitted before your student can attend school.
- MINIMUM VACCINE REQUIREMENTS BY STUDENT’S AGE
- Infant - 4 Years
- - 3 doses Polio vaccine - one received on or after 4th birthday
- - 4 doses DTP/DtaP - 4th dose not required until 18 months
- - 1 dose MMR vaccine - received on or after 1st birthday
- - 1 dose Haemophilus Influenza (HibCv) - on or after 15 months unless a primary
series and booster have been completed
- - 1 dose Varicella vaccine - on or after 1st birthday or physician/parental confirmation
of the chicken pox disease
- 5 - 7 Years
- - 3 doses Polio vaccine - one received on or after 4th birthday
- - 4 doses DTP/DtaP - 4th dose not required until 18 months
- - 1 dose MMR vaccine - received on or after 1st birthday
- - Measles Booster - have a 2nd dose of measles vaccine
- (may be received as part of second MMR)
- - 3 doses Hepatitis B vaccine by 5th birthday
- - 1 dose Varicella vaccine - on or after 1st birthday or physician/parental confirmation
of the chicken pox disease
- 8 - 12 Years
- - 3 doses Polio vaccine - one received on or after 4th birthday
- - 4 doses DTP/DtaP - 4th dose not required until 18 months
- - 1 dose MMR vaccine - received on or after 1st birthday
- - Measles Booster—students born on or after September 2, 1991,
Must have had 2nd dose of measles vaccine by their 5th birthday. Students born between September 1, 1978, and September 1, 1991, must have had 2nd dose of measles by 12th birthday. Booster may be received as part of 2nd MMR.
- 13 - 16 Years
- - Td (Tetanus Diphtheria) Booster
Check with personal physician to be sure all vaccinations are current.
